Plusnet reps here have occasionally dealt with similar, that turned out to be "an imbalance" in the BT Wholesale system. They were able to get it sorted out.
The nearest theory we have got to what they mean is that the BT IP Profile is held in two places, with the second (that does the actual throttling) being updated from the one that is reported by the BT speed test, and that the update to it sometimes fails in the same way as the Plusnet update does. Probably not for the same reason, but the same effect.
