Local exchange is Laurencekirk - small residential village, and I believe the upgraded service goes live at the end of November 2012

Currently on ADSL Max package with BT Broadband, syncing at 7mbs, as we are less than 1/2 mile to the exchange, in a modern development - nice steady connection, with no disruption
My question is that whilst I assume there is a potential speed gain with the 21CN upgrade, what sort of real life gains can be expected, and what, if anything, do I need to do with BT to upgrade the account?

We'd need your router stats to be able to say; everyone's different.
Don't bank on that date. These are planned dates and have been known to slip often.

The stats make it look like you're next door to the exchange! You should receive the full 24 Mb/s sync.

You're right on top of exchange. On 21CN ADSL2+ you should get the full 24 Meg Down and about 1.2 Meg Up.

Encouraging
Once the exchange upgrade is complete, what do I need to do, if anything, with BT Broadband to get the speed upgrade?

They will nomally upgrade you automatically at no cost, but not straight away. It's cheaper for them to have their customers upgraded in batches when convenient for Openreach scheduling.
You can get an earlier upgrade, but most ISPs are charged extra by Openreach for that, and pass that charge on, about £11 I think.

All the post needed was, somewhere in it, to justify the opening statement, was "In my experience". Then it might have been useful.
As it is, it looks more like a hijack (as suggested by XRaySpeX), than a contribution. I also believe it is incorrect and that the poster was misled. It is automatic for most ISPs, and I am fairly sure BT is one of those.

The attenuation shown there is 32.3dB.
That'sa fairly poor sync for that attenuation. I suggest a reconnection during daylight hours and then take the stats immediately after and post those. The noise margin looks high.
You should have a downstream connection speed of more in the 13-14Mbps range.

Exchange went live a couple of weeks early - there was an email from BT early last week saying the upgrade date was going to be the 16th November
Line is still settling down, but this is the difference so far...
Download speed increased from 7936 to 14374, and upload speed increased from 448 to 1251

Love the concept of settling down.
I'll just say if you think your sync speed is going to increase over a few days (i.e. at end of the 10 day period) I will be very shocked.
it looks to have decided on a 15dB target already, slowing you down from optimal, now this may be the result of errors not seen in those stats. This settling period goes on forever, so speed may improve in a jump in a few weeks time or it might not.

Update - connection has been nice and steady for the last couple of weeks, and downstream has seen a steady increase in sync rate to the present 18 megabit, whilst upstream has been consistent, as expected
