Hello,
Anyone know which adsl2+ modems support this config? (perhaps with 3rd party firmware, if necessary)

The DrayTek 2820 series and the newer 2830 series will do what you want.
The older DrayTeks, like my 2600 series, will do the mixed DMZ and Nat, but not ADSL2+
Line One:- Zen - Dray

Line Two:- EntaNet (Aquiss) - Dray

DrayTek have confirmend the 2710 series also do this, so I've ordered and 2710dvn.
