Although it obviously doesn't apply here, there is an issue with all Macs running OS X Mavericks using a Broadcom 43xx wifi chipset that causes high pings of this order. (I am a beta tester for the new driver, the last two iterations work flawlessly for me, so this should be released soon as an Apple update, probably 10.9.2 packaged)

Is that just the ping to the router, or is it added to all subsequent pings?

It's a genuine increase and variability in response time to the access point, so adds to all LAN or WAN pings. It is only seen after inactivity of traffic, but this period is extremely brief - a standard ping wait time exceeds it, a standard traceroute wait time doesn't (ie a repeated ping shows all high numbers, a traceroute shows a high number for the first packet only, all numbers would be fine if a download was in progress first).
