Openreach sent me a link to this form
https://openreach.co.uk/fibreformybuilding in response to an enquiry I made. This is an extension to the usual "register my interest" page and it's collecting landlord/agent details to enable wayleaves etc. to be obtained.
I think at the moment they are only interested in capturing this information where the building is in an FTTP deployment area, but it probably can't hurt to submit details anyway if you're in a flat.
