What on earth is this failure mode?

I'm not as familiar with FTTP as I was with DSL, so my usual assumptions (SMPS attached to a timer set to 100 minutes) don't really seem to apply here.

Well, CityFibre popped around today, swapped out the ONT for a different (apparently newer) model, issue resolved. I'm still entirely none the wiser as to why or how, and the chaps from Kelly Comms didn't seem to have a clue either, their exact words were "all we do is fit these, no idea how it all works".
