The last engineer to visit for a voice fault didn't even have a PQT tester, and he also implied that most OR engineers don't carry them as they have only started being issued them, so if they are supposed to be carrying out both tests and they are not how are they supposed to find faults ?

How long ago was this ? Even before the, now common, use of HHT's (hand held terminals,Exfo or JDSU,) then engineers were expected to use their Hawk and laptop to run a pair quality test. I am aware that some engineers were not doing this as standard, but it is mandatory for any one carrying one, and many now do. The Eclipse test is a dial in automated test service, that if successful confirms the correct DN and that the pair meets SIN 349.
There are plenty of ways to fault a line without the use of a Hawk/Exfo/JDSU.

Last engineers visit was during March-April 2012, apart from the Blue handset (basically a telephone ) and his mobile phone he had nothing ,the line was installed in 2006-7, As for those remote tests they are not that reliable , although the new CIDT test is allegedly better than the older ones,
I am not saying a engineer cannot locate a line fault without all the test kit, but it was you who said that all engineers should or do carry out those tests that you specified , my point was that in my experience they don't, and that can and probably does lead to some of them just saying "right when tested" ,when they can't be certain that is the case,or be bothered to do their job properly, meanwhile bt retail charge the customer £130 for the privilege ,unless the customer challenges it and complains about it
