Router not connecting

I have a Linksys WAG54Gs v1.1 which has been working using my TalkTalk connection for the last couple of years. However yesterday I discovered that the router had some how been reset to its factory settings. I have tried to put the settings back to what I thought they were but I am unable to connect to the internet. The Internet LED doesn't light and when I login ti the router settings the Gateway (i.e. Internet connection) is showing as not connected.

I have tried connecting using the TalkTalk supplied modem and that connects fine, so I assume that the problem lies with the router. Could somebody please post the settings that I need to put in. I think that I have tried most combinations without any success.
