Looks like a nice 700 series telephone with a BT 431A line cord modification?

Edit: My vote goes for push button, can't believe anyone would be stupid enough to be using 'loop dis' for dialing out.

Don't forget that the first push button models of that phone were only loop disconnect too. In fact, I found one that looks identical to the one in the picture in a cupboard at my mother's house about three weeks ago. It's 1981 model. DTMF models came along later.
