Web Proxy via HTTP Portal Page

Tricky working out where to put this!

Okay so what I want to do is to have a proxy server running at home, however I want to encapsulate it inside an HTTP portal in a similar manner to the various sites on the Internet which offer this service like Hidemyass and others.

Any off the shelf solutions?
