I remember Kelly (Plusnet ISP) said that it wasn't necessarily an overloaded Plusnet gateway that exhibits these anomalies, but perhaps the route an individual's connection is routed, over the BT Openreach network, to that gateway.

Wrong! The DLM monitors sync issues. There is no loss of sync so it isn't seen by the DLM.
