VDSL2 above 1.5km is hit and miss.
You missed out a very important option, for long EO line clusters the FTTC cab can be installed close to the cluster and the fibre run back to the exchange can be many many miles long. Cornwall is doing this for EO, and/or using FTTP.
Fibre to the DP (G.fast) I would not bother worrying people yet, as BT has talked about it, but more at the research and decade long time scale.
