Your ISP can do a live stream of a TV program to your exchange, and all that ISP's customers on that exchange can watch that program live. Before multicast your ISP had to do a stream for each customer. The stuff in the exchange "multicasts" it out to them.
AIUI, FTTC multicast does it at the exchange then cabinet levels.
